Rachel Hernandez (she/they), MS, LPC, has over ten years of experience in providing individuals with case management and mental health counseling. She has worked in a variety of non-profit organizations in both Chicago and Madison. She is a strong believer that individuals are the experts of their lives and are incredibly resilient. She is honored to have been a part of many individuals' journeys towards healing. She continues to be amazed by folx's ability to overcome life's most intense adversities. In providing support, Rachel pulls upon Person Centered, CBT, Trauma Informed, and DBT approaches. She also believes that it is very vital to honor and explore how an individual's intersectional identities impact how they navigate their community and systems.
In practicing self-compassion and self-care, Rachel enjoys spending quality time with her partner and two young children. In her free time, she enjoys going for walks in nature, gardening, jamming out to music, and being crafty.

Individual Skill Development
Individual Skill Development and Enhancement
Individual Skill Development and Enhancement (ISDE) with Hope Inspired, for up to 12 hours per month, could include teaching, modeling, training and practice in communication, interpersonal skills, problem solving, decision-making, self-regulation, conflict resolution, and other specific needs identified in your service plan. Services may also include training in daily living skills related to personal care, household tasks, financial management, transportation, shopping, parenting, accessing and connecting to community resources and services (including health care services), and other specific daily living needs identified in your service plan.
Services provided to minors also focus on improving integration into and interaction with the minor’s family, school, community, and other social networks. Services include assisting the minor’s family in gaining skills to assist the minor with individual skill development and enhancement.
Individual Family and Psychoeducation - Managing Mental Health/Substance Use Concerns
Individual and Family Psychoeducation services with Hope Inspired, for up to 12 hours per month,
